This is a hands-on supervisory position overseeing the delivery of Extended Foster Care (EFC) services to eligible young adults who have transitioned from licensed foster care ages 18-21.
These services include but are not limited to life skills training, community outreach, referrals and system navigation to ensure that the emotional, physical, behavioral and mental health needs of young adults are met.
In addition, this position will ensure that appropriate living arrangements are established that will allow for the young adult’s personal development while providing necessary support, guidance, and continued education and employment.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.

Attend, complete, and demonstrate competency in all required Camelot training.
Develop, mentor, coach, and supervise assigned staff to promote client safety, well-being and a successful transition into adulthood in accordance with all applicable contractual and regulatory requirements.
Identify and analyzes problem areas in order to improve quality of care.
Monitor and assess the implementation of interventions being utilized by staff in the program.
Evaluate assigned staff competency in all job requirements and provide regular feedback and corrective action when necessary.
Provide supervision according to Camelot policy to staff and document follow-up and progress from previous assignments.
Review all program applications and all monthly progress reports as required to verify program compliance of participants.
Review and submit weekly/monthly check requests.
Ensure all home visits and documentation are completed on time by staff and in compliance with policy and procedure.
Ensure completion of quality documentation and documents are submitted or filed timely.
Monitor and report on contractual outcomes related to the completion of Transition Plan Addendums, Case Plans, Judicial Reviews, EFC Initial Program Applications and Readmission Applications, Voluntary Placement Agreements, Shared Living Plans, Supervised Living Arrangement Assessments, Participant Budgets, Housing Partnership Agreements, and any other relevant program documents.

Monitor and follow-up with staff regarding scheduled court hearings to ensure proper attendance/coverage, the outcome of court hearings and documentation in FSFN.
Attend court hearings to provide support to staff and/or young adult as needed.
Coordinate and/or conduct client hands-on training for skills necessary to live independently.
Training will consist of at a minimum budgeting, nutrition, family planning, education planning, job training, obtaining and maintaining housing and employment and ensure that service delivery is community based allowing for experiential learning opportunities for young adults.
Facilitate conferences/meetings in effort to prevent placement and/or program disruption, service interruption, or termination.
Facilitate emergency staffings related to young adults encountering crisis situations.
Provide 24/7 on-call crisis support.
Collaborate and create single points of contact with health, mental health, educational liaison’s, employers, and other youth specific community services and supports to streamline access to resources, care, and services.
Advocate for greater access to affordable housing, public transportation, health, educational, and employment opportunities for participants.
Recognize, respect and respond to the unique, culturally-defined needs of others served.
Meet budgetary expectations through utilization management of staff productivity, client authorizations and eligibility, and assuring proper staffing according to the flow of referrals and number of clients in the program.
Assist Director of Independent Living Services with community activities and presentations.
EDUCATION and/or EXPERIENCE
Bachelor’s Degree in a Human Service discipline and three years of related child welfare case management or experience working with children/adolescents and families.
Master’s degree with Two years of supervisory experience preferred.
The number of years of experience and exact type of experience shall be in accordance with COA and applicable contract/funding requirements.
